(3) Addressing Social Determinants of Health through NC CARE 360 platform creating comprehensive referrals for housing, food, employment, legal, and healthcare needs
(4) Peer Support, Community Engagement, and Social Connectivity through Health Ambassadors with lived experience,
5) Community Building and Support Networks through establishing peer support groups and embedding recovery support
TRY’s This comprehensive approach recognizes that effective intervention requires understanding communities' direct context and needs while addressing root causes of disparities including racism, poverty, geographic barriers, and mistrust of healthcare systems.
